Vital Records
State law requires all customers (U.S. citizens or immigrants) to present identification, proof of date of birth, and social security number before a driver license or identification card can be issued.
If your vital statistic record is not available application for a duplicate may be made via the Internet. For your convenience, the Tax Collector’s office provides these helpful links.
